Physical therapy to help with pain, especially shoulders. And please see your doctor for a referral for a sleep study. Sounds as if you may have sleep apnea which not only disturbs sleep but has many other effects.

The chronic shoulder problems could be due to rotator cuff dysfunction and might require orthopedic consultation. If you have true REM sleep disorder, this could be the first sign of impending Parkinsonism or Lewy Body Disorder, and would need a neurological evaluation. Sleep apnea may be a more likely problem. An overnight sleep study could answer questions.
