There are a few possibilities, but two of the most likely are myelodysplasia (a cancer-like condition of the bone marrow) and chronic myelogenous leukemia (CML). He should see a hematologist/oncologist, who will be able to make an accurate diagnosis with just a few tests. Good luck to him.
