There are a few possibilities, but two of the most likely are myelodysplasia (a cancer-like condition of the bone marrow) and chronic myelogenous leukemia (CML). He should see a hematologist/oncologist, who will be able to make an accurate diagnosis with just a few tests. Good luck to him.

Chronic bleeding and malignancy or cancer come to mind first in this context. Best will be to have his ordering doc comment and further coordinate in context.
