Unclear as to extent of damage 5 yrs ago, but your current description involves at the least chondromalacia, and perhaps wear and tear beyond this. Whether the ligaments are intact is also a question. See an orthopedic surgeon, get this evaluated, discuss various therapeutic approaches.
