I'd say get this checked in person. Could be something as simple as a little milk residue, but could be as bad as a certain type of cancer. Get it checked. If the piece falls off hang on to it and take with you to the appointment.

Could be normal shedding or ductal effluent. Trauma to nipple? Traction of clothing? I would not be concerned
