Pubertal gynecomastia occurs during puberty if at all not at age 20yrs. No it does not cause prostatic enlargement. Self diagnosis is dangerous let an MD check you if you have gynecomastia

One half of adolescent males experience physiological gynecomastia due to the 1. An increase in estradiol 2. Lagging free testosterone 3. Increased sensitivity to normal male estrogen It should resolve within 2 years of onset or before 17 years of age. If it persists then further evaluation is needed. Gynecomastia may have shared a common cause for prostate change, not effect. Hope it helps.
