Painful "external piles" may represent hemorrhoids with herniated blood vessels within, and the pain from that may also come from Herpes I/II sores in the peri-anal region. See your gyn for an IgM/IgG test for Herpes, and an exam to be certain what you are experiencing is b/c of actual hemorrhoids.

The best way to deal with this problem is to go to a gastroenterologist or general surgeon who deals in anorectal issues. You may have a persistent hemorrhoid that needs surgical treatment or other medical treatment.
