Progression: Appendicitis usually progresses over hours from a dull and vague central discomfort to an increasingly more intense and constant localized right lower quadrant pain. Eventually the pain becomes intense enough that you don't want to move, jiggle, cough, laugh, or even deep breathe. Often there is low grade fever and chills. Constipation is usually more diffuse and has none of the aforementioned.
