An anal fistula may or may not be related to a Crohns. Individuals with other colorectal disorders such as fecal incontinence or hemorrhoids can have them.

Fissures occur in the midline, either anterior or posterior. The lateral lesion sounds more like a fistula. Both are fairly common in the general population without Crohn’s disease. Constipation or hard pelletized stool from lack of adequate non caffeine/ non alcoholic fluids. Best to get an accurate diagnosis/treatment and see a colorectal surgeon.
