Contact your physician, as there could well be structural problems in neck or shoulder. However, it would be of value to rule out any cardiovascular issue which is causing referred pain. Intervention can be based upon outcomes. Do this soon to be on safe side.

It's difficult to answer this without a physical examination.I would see your general practitioner or primary care doctor. This can be from the neck from disc, muscle, or nerve problems, or referral pain from organs like the spleen, stomach, lung, heart.
