Many things...: Possibilities include pregnancy, tubal problems, endometriosis, ovarian cysts, infections, colon issues, diverticulitis, tumors, etc, muscle strains, herniae... You should see your gynecologist who will do a thorough exam, take a good history and maybe order some studies.
