Acid reflux: Bad breath with good oral hygiene is from acid reflux..Too much caffeine from coffee soda or tea can be a cause. Medication and otc tablets are recommended with a decrease in caffeine.

Halitosis: Bad breath can be caused by several things. One, is tooth bacteria, but your tounge has crypts that have bacteria as well. Tonsils, and acid reflux can also cause bad breath. Brushing your tounge and watching what you eat can be helpfull. Speak to your dentist and maybe a physician if you suspect reflux.
