Possible fungus : I will give you credit and say this, you sure tried all the available options to try and treat it except one. That being over-the-counter antifungal medication. If that is available to you Lamisil is what I recommend. If your feet perspire a lot this also needs to be addressed . If you do not respond to this please seek medical attention
