Dermatitis: You didn't indicate if child is crying from the swelling! Best guess is that from wetting self and being in a damp diaper an irritation of the skin will cause a redness and a swelling of the area. Make sure to change diapers frequently and use a desitin like substance to protect the skin area. Hope that helps.

Idiopathic edema: This is bluid retention in penile skin. It is usually painless and resolves spontaneously with 36 hours, espeially in circumcised boys. Penile skin in uncircumcised boys can swell with infammation under foreskin and head of penis. Responds to antibiotics &/or Bacitracin or Neosporin ointment.
