My wife has a lump in her breast and need to see a dr. do i get approval/ auth or how does it work?

Consultation a must: Certainly, wife needs to consult health care professional. Call her primary care physician, if she has one and request referral & be advised. Call health insurance if no PCP. Call for advice from local surgeon's office if no PCP or insurance. There may be local specialist who provides free consultation and/or care if no insurance and financially strapped.
