Does the doctor have to tell you if you are insured or not?

Insured versus not: This question makes no sense. Your doctor has nothing to do with whether you are insured. If you have insurance then you have contracted with them to have them pay for your medical care to some degree. If you have no insurance then the payment for the medical services will be billed to you. The doctor's job is to provide appropriate and excellent health care regardless.
