Hernia: He needs to be examined. It could be a hernia which is not uncommon at this age. Can you differentiate from the testicle and scrotum? Scrotum will get bigger when he cries or cough if hernia is the problem. Otherwise, if the testicle is warm and tender, epididymitis may be present.

Testicular swelling: There can be swelling of the testicle with pressure or from twisting of the testicle. It is probably best for him to be seen. An ultrasound of his groin will let you know if there is a difference or problem that is causing the swelling.
