GP: Your GP should be able to help with note or the physician treating your depression if not your GP. For a legal medical note the entity completing it will have to gain access to your medical records.

STILL NEED DR: No one can give you a medical excuse other than your doctor. Not sure of your options. Perhaps just ask family or friends for funds to get into doctor and get a note.
