No: Throughout history, medical notes were meant for other medical professionals to read. Thus, they could contain all sorts of medical abbreviations, and they could be just a collection of phrases. Nowadays, notes come in a types. Some are meant to be readable by non-medical staff, and some are even generated by computer templates to make note-writing go faster.
