At obgyn exam, paper on table tears beneath me and my bare bottom/genitals are exposed to the uncovered table. what are the chances of picking up trichomoniasis (or other stds) from exam table if it was not properly cleaned from previous patients?

Trichomoniasis: Low because one would not anticipate that anus or vagina would actually come in contact with the surface. It is more likely to be skin touching exam table.
