Neck and shoulder : Sound like you need a clear diagnosis first. You should see a doctor of physical medicine and rehabilitation. The question is if you have a nerve problem in the neck as one of the root causes of this.

Start with GP: Start with your GP who can sort it out based on findings. He or she can further coordinate referrals as needed. Referral letters cannot be provided on this forum.
