External vs internal: Creams are better for true external hemorrhoids, however internal hemorrhoids may prolapse to outside but are true internal hemorrhoids. In the latter case best to attempt to gently reduce the hemorrhoids to their internal position using cream as lubricant and then use suppository or foam/ cream internally. Finally fix the problem: constipation by increasing fluids/fiber and decreasing caffeine.
