Spinal Neurosurgeon: Urgent items (emergency room evaluation) if the pain is severe can be for heart attack or aortic dissection. For intermittent and chronic repetitive symptoms, this sounds spinal in origin. You would best see a spinal specialist like a neurosurgeon who may order imaging tests after a complete history and examination.
