I have been suffering from a severe pain on my left side of my neck and shoulder for 2 days now and it extend to the head should i be worried.

Pain: This could be musculoskeletal. Have your tried heating pad (don't get burned), warm shower water directed to area or NSAID's. At this point, it may not be time to worry. However, if it is not resolving, please do consider medical eval.

Neck and head pain: This should be worked up and you need to be seen by your physician. Temporal arteritis can cause pain in this area and this disorder can lead to blindness if not treated. Brain hemorrhages as well as a lo!ng list of benign conditions can also cause this type of pain. Don't wait--see your physician. Good luck!
