Basics: The barium itself is inert & there are no sensory endings within the gut (like those on fingers) that would sense pain. Bowel pain is triggered by stretch/pain fibers when material of any sort moves & stretches various areas. Bottom line, if anything stretches the gut wall be it air/water or barium it may be felt as pain.
