Yes: Since most patients actually are either asymptomatic or recover quickly, it is reasonable to assume that the epidemic went undetected for a while until somebody got infected with a very virulent viral strain/mutant or developed severe disease due to underlying other debilitating condition

It is possible: There could have been small numbers - especially if it was in a less virulent form.
