Can a stroke occur years after severe strangulation? i read that clots from artery damage after strangulation can cause a stroke. can this occur even 4 years after the incident?

Unlikely to be old: clots at this point. Possible old scar tissue could give pain/discomfort. An exam with your doctor to be sure is recommended.
