What is the problem if one eye pupil is smaller than the other?

Many things: This could be physiologic, meaning it's benign and just the way the pupil is. This could be from getting a drug in the eye, a neurologic condition, test inflammation in the eye, and rarely brain aneurysms, brain tumor, cancer, and past inflammation, infection or ischemia in the optic nerve. If this is new you should go to your doctor.
