Migraines neck pain: The smallpox vaccine can have many side effects including headache and stiff neck, however these are uncommon. Given the potentially dangerous consequences of these side effects it is suggested that a doctor evaluate these in person. https://www.mayoclinic.org/drugs-supplements/smallpox-vaccine-injection-route/side-effects/drg-20071296

It is not normal to: It's not normal for any regular person to get the smallpox vaccine now because smallpox disease was removed from Earth before 1980, after decades of vaccinating the world's population. A person's infectious diseases specialist, along with his primary care doctor, can evaluate to see what is going on, if he is a very special person who truly got smallpox vaccine (such as for work-exposure reasons).
