I get a tightness in my chest making it feel difficult to breath, usually exercise such as walking worsens it and causes it to be even harder to breath. the discomfort makes it difficult to sleep. feels like my heart's being squeezed.

Chest discomfort: I recommend consulting a cardiologist. Chest discomfort relieved by leaning forward is characteristic of pericarditis which is an inflammation of the heart lining often caused by autoimmune disease. Chest distress aggravated by exertion is seen in coronary artery blockage but it would be most unusual in your age group.
