Start with rinse: I would stick him in warm bathwater & rinse the area as the first step.Cool water over the site will also help & a gentile effort to retract the foreskin can reveal trapped debris which could trigger such a response. If you find that he does have colored drainage from the pee hole itself I would have him seen.
