Details matter...: Question is vague. When a couple are having sex, the penis should be swollen, and of course hard enough to easily go in and out of the vagina. The clitoris should be swollen too, but it is a smaller thing and sometimes not so obvious. Swelling relieves upon orgasm. Bleeding can happen from small tears in the hymen if the hymenal opening was small or the technique was not gentle enough, etc...
