Odd post: A toddlers testicles are inside the skin & how you would decide they are hard/sore is unclear. If the skin of the scrotum is inflamed by diaper rash I would expect the kid to be unhappy with any effort you made that touches the area. In normal care the kid only needs a cursory washing of the area & no creams/lotions need be used. If the issue troubles you have the kid seen.
