Skin infection: He may have a diaper dermatitis or staph skin infection, please see your Pediatrician. Change the diaper often, keep the area clean and dry with the use of a gentle soap that has no fragrance, apply diaper cream with changes. A topic antibacterial cream may be applied three times daily until seen.

Time to see doctor: Testicles are very hard... is not how toddler's testicles tend to be. Thus, it's time to see his pediatrician for an exam. The doctor will decide where the problem is... such as in the testicles, something else in the scrotum, or the scrotum itself. Doctor will ask when symptoms started, how they progressed, any use of diapers, hot baths, soaps or creams, etc...
