Consult doctor: This isn't common but the severity of it depends on your age, med hx, etc. Consult your general doctor or urologist as soon as possible. The hard area on the scrotal sac may have nothing to do with the pain or may be an infection. Testicular pain can lead to permanent damage of the testicle.

No: Occasional pain in the scrotum that doesn't last is usually not concerning. The lump may be ok too but you need to see a doctor for an exam and maybe a scrotal ultrasound.
