Dental Prosthetics: It is unknown if your dental coverage would pay for a remake within 2 weeks, but my guess is it will not. Your best course of action is to address your concerns with your restorative dentist. Dental prosthetics are custom made and you should be happy with your smile and appearance.

Usually not: Most insurance companies, including government funded healthy care, will only pay for 1 restoration of a given tooth/teeth over a specific period of time. You can call your carrier, present your problem, and ask. Probably, however, your best bet is to talk to your Dentist, express your dissatisfaction, and ask what can be done.
