Not STI: Such symptoms rarely are due to STI, and you cannot catch any STD from having a massage, including hand-genital contact. Most symptoms like this are psychological, and continuing symptoms despite azithromycin is strong evidence against infection; your 2nd STI tests will also be negative. Stop trying to treat and probably stop testing for STDs.
