Check with provider: His blood pressure sounds a little low. Given the pacemaker has recently been replaced it would be less likely a pacemaker problem. However, is his pulse OK? He is diabetic and how is his kidney function? I am sure he is on BP meds and if kidney function worsening he could be getting low BP from meds. I would let your cardiologist or PCP know about the low BP
