Time to see doctor: Breast pain, nipple inflammation, breast firmness or lump, etc..., found in a woman's breast in her late 30's should be examined by her primary care doctor. If cancer is suspected, special studies can be done. If the problem is found not to be cancer (such as a benign nipple or breast skin problem), her doctor can offer advice and treatment too.

Likely mastitis: Inflammation of breast and nipple with infection spreading to lymph glands under armpit. Most likely require antibiotics. Should see health care provider. Breast will need to be examined after inflammation settles down to check for possible malignancy which might co-exist.
