Locally, carefully: "Who do not care for" means 5 different things to 10 different people, so it helps to discuss the situation with a trusted friend or relative first, especially if one has a trusted person in the medical field. A patient may need to continue getting care if she has an ongoing condition. That means reporting the "only doctor in town" may not be a good idea until after she is seeing another doctor.

Ask locally: Most of the docs answering questions on this site are in the USA where such reports go to the state licensing board. In my state there is a call in number to start the paperwork. How it is done in South Africa is unknown to me but is likely similar. Contact a local public health entity to see if they can guide you in this process.
