See a physician: Unfortunately this is hard to diagnose without a physical exam and seeing the shoulder, so I would recommend you see a physician for evaluation. This could be cellulitis (inflammation of the skin) if the skin is red and hot. This could also be a muscle or tendon issue or something else entirely.
