Need further w/u: According to the composite score of main reflux symptoms, the diagnosis of GERD can be made without further tests in most cases. However, 24-h esophageal pH monitoring and Upper (EGD)endoscopy are still indicated in patients with atypical symptoms.I would consider other dx inspite of +ve PH test. R/O Erosive gastritis or ETOH, or H.Pylori induced ulceration/gastritis causing gnawing pain. SEE GI
