Referred pain: All doctors learn that the location of perceived pain may be different from where it is actually being caused. It has to do with the organization of the nervous system. To oversimplify, esophageal pain may be referred to the shoulder via the adjacent diaphragm. So the problem isn't in your shoulders, it's in your GI tract. See your doctor.
