Possible TMJ : syndrome which will have most of the features you describe. You must be seen by a dental expert on TMJ issues.

Joint dysfunction: Sounds like the cartilaginous disc in your jaw joint is out of place. Important that you bed seen by an Oral Surgeon immediately to see if it can be repositioned.
