Collarbone: Soft tissue swelling above collarbone would commonly be a lymph node. Sometimes cystic structures or even a lipoma or something of that nature can present.You should be evaluated.

Puffiness: I agree with Dr. H. Occasionally we see harmless puffiness or tenting of the supraclavicular space, especially in cases of emphysema.
