Good question: It is known that large platelets have more dense and carry more secretory granules. This has been associated with higher risk of vascular problems. However, it is not known why. In a cardiac article from 2012 "Nature Reviews Cardiology" the author recommends further study. I could not find a good physiology article on this. However, perhaps another physician does.
