My 4 year old has had a very bad cough for about 4 weeks - she was given nasal spray, tablets and syrup as she was diagnosed with very bad nasal issues last week - we were told that it should be better within 4 days, but it's gotten worse this mornin?

Worth a re-check: Cough & cold symptoms may be complicated by ear or sinus infections as they work through their system. Symptomatic treatments seldom do much more than give you something to do. If you feel the kid is worse, it is worth a re-check to see what else is going on.
