How can a dentist tell irreversible pulpitis vs. reversible pulpitis after tooth filling. i had a feeling 4 wks ago & still have hot/cold senitivity.

Pulpitis: Inflammation of the nerve inside a tooth. If it's reversible, then the sensitivity will abate over time (varies widely between patients, no set number). If the sensitivity continues at the same level for some time (say, four to six weeks) then the chances are it's irreversible. If the sensitivity begins to get worse and not better day after day good bet it is irreversible. Was your filling deep ?
