What is the next step when breast cysts are painful and doesn't improve after treatment. it's been painful for a month?

What treatment: If your primary and or OB/Gyn treated ane I assume did Ultrasound and or mammogram to rule out mass/abscess then the next is to see a Breast surgeon to remove or aspirate the cyst.
