I am a runner who was out for 16 months with injury. upon return my hr was average 150 but recently has been close to 200. hr has never been this high?

The maximum heart rate during exercise that a persons system can reasonably tolerate is ~220 minus your age in years. Your post suggests you are out of shape and pushing yourself beyond a reasonable point for your level of rehab or you have an underlying problem. Slow it down some and see your doc before you continue at this pace.
