What could be the cause of low acth but normal cortisol? acth is 4.9. cortisol total is 8.9 and cortisol free is 0.33

If there is elevation of circulating cortisol by exogenous or endogenous factors, the acth level will be suppressed by a biological feedback mechanism. I'm unsure whether this explains your situation. An endocrinologist is best qualified to answer your question.
